Eligibilty Requirements
- Individuals 18 years of age or younger with Medicaid, insurance that does not cover vaccines, no insurance coverage, or of American Indian/Alaskan Native descent
- LCHD will request an administration fee of $22.00 per injection
- Vaccines will not be denied due to an inability to pay
- LCHD does bill some insurance companies
- Parents should call to verify that LCHD accepts their insurance
